Branded pens follow a 0.25 → 0.5 → 1.0 → 1.7 → 2.4 mg weekly ladder. Compounded vials need the label concentration before any draw. STRAND will not auto-escalate.

Reconstitution note
Compounded vials require a user-entered mg/mL. Never infer concentration from the name. 2.5 mg at 10 mg/mL is 25 units, not 2.5 units.

FDA has documented 5–20× overdoses from mg ↔ units confusion on compounded products. Always show mg + mL + units together.
